Yesterday, the Transform Advanced Kidney Care (AKC) initiative brought together a diverse group of healthcare professionals, patients, carers and charities for a productive Patient Information Meeting in Birmingham.
Representatives from the five Level 3 pilot sites - Bradford, Royal Free, Stevenage, Nottingham, and Kilmarnock - were in attendance, joined also by colleagues from Oxford and Portsmouth to share insights and best practices.
The day was packed with discussions and collaborations, focusing on:
- Collaborative updates from the Transform AKC programme.
- Sharing patient-provided AKC information to foster mutual learning.
- Identifying barriers, challenges, and solutions in advanced kidney care.
- Developing key resources, including:
- A checklist for what patients with eGFR 12 should know.
- A checklist for the essential information patients should take away from their first AKC clinic visit.
- Hearing from pilot sites on their current AKC processes and areas for improvement.
This meeting highlighted the power of collaboration, with attendees exchanging ideas and working towards improving AKC pathways and resources.
