Dialysis at Yours: A home dialysis improvement hub

The Home Dialysis improvement hub

"In order to make our service sustainable for the future, we must listen to patients, invest in what will enable them to dialyse at home, and have the infrastructure to support the patients once they are there..."

Improvement essentials

Interventions or changes may require certain factors to be in place within a kidney centre or team to be successful.  We think of these as the 'DAYLife improvement essentials'.

Click on the boxes in this section to learn about how the DAYLife improvement essentials - data, leadership, patient involvement and staff education - can help home dialysis teams achieve successful improvement.

Improvement opportunities

This section of the hub shares improvement activity and learning from the home dialysis quality improvement project DAYLife, as well as other recommended national resources and information.

The learning is split into sections of the patient pathway to home - from pre-treatment decision making, to support and care for those receiving dialysis at home, as well as the many points of kidney care that happen between, before and after.

Click on the boxes to read patient and staff experiences, case studies, tested interventions and shared learning along the patient pathway to home.
